Worker hurt at bridge site

A worker at a bridge-reconstruction site in North Annville Township was seriously injured yesterday afternoon when a piece of equipment struck him, a Penn-DOT spokesman said.

The accident happened on the first day workers were at the Clear Spring Road bridge site, Penn-DOT spokesman Greg Penny said.

An employee of Clearwater Construction of Mercer in western Pennsylvania was injured when he was struck by the bucket of a backhoe, Penny said. He was flown by Life Lion helicopter to the Hershey Medical Center. His name and condition were not available last night.

Efforts to reach a Clearwater spokesman were unsuccessful yesterday.

Workers were clearing brush and trees from the site in preparation to replace the bridge over the Quittapahilla Creek, Penny said. The bridge is located about a half-mile north of Route 422.

“Its been a while since I’ve heard of a serious injury in a work zone,” Penny said.

Bellegrove, Annville Union Hose and Palmyra Citizens fire companies, First Aid and Safety Patrol ambulance and fire police were called to Syner and Clear Spring roads near the entrance to the Pennsy Supply quarry shortly before 1:30 p.m.

Clearwater, located in western Pennsylvania, was awarded a $1.286 million contract to replace the bridge. The project is funded with federal and state funds, Penny said.

Penny said PennDOT is not involved with the bridge replacement but has a contract with a private inspector for the project.

3 dead : 5 injured at tunnel construction site

Three workers died and five were injured when they were buried in a landslide Monday at a tunnel construction site in the city of Hwaseong, just south of Seoul, police said, according to Yonhap News.

Kwon Tae-won, 55, Choi Jae-mun, 59, and Kim Bok-hoi, 58, were found dead hours after the 7:18 a.m. accident, in which the arch of the tunnel collapsed due to an apparent weakening of the earth caused by recent rains.

Forty people were working at the site, but most of them escaped the heaps of falling rock.

The five injured workers were taken to two nearby hospitals.

Jeon Hyun-young, a civil engineer rescued 35 minutes after the collapse, sustained fractures on his legs.

The tunnel, measuring 50 meters high, 50 meters wide, and 240 meters long, is being dug through a mountain to accommodate a four-lane highway.

Construction worker dies at Taum Sauk

A woman was killed Tuesday while working on a concrete placement as part of the rebuilding project at the Taum Sauk reservoir, which collapsed more than three years ago.Ozark Constructors LLC said the 48-year-old worker was flown to Barnes-Jewish Hospital in St. Louis, where she died. Her name has not been released.Ozark Constructors had been working on the rebuilding project at the AmerenUE-owned reservoir in southeast Missouri. A breach on Dec. 14, 2005, sent a wall of water down a mountain into Johnson's Shut-Ins State Park.

The company said it was making grief counselors available to employees and their families. St. Louis-based AmerenUE said the safety of everyone at the site is the company's top concern.

"Our thoughts and prayers go out to the family and co-workers of the construction worker who died as a result of this morning's tragic accident at the Taum Sauk upper reservoir rebuild site," the utility said in its written statement.

AmerenUE said ASI Constructors Inc., a business that specializes in dam construction, and St. Louis-based Fred Weber Inc., a construction company, were venture partners in Ozark Constructors LLC.

Ozark Constructors said it has put in more than 2 million hours of work at the site over 21 months. It declined to release additional information, pending an investigation by the Occupational Safety and Health Administration.

AmerenUE expects the rebuilt plant at Taum Sauk will begin generating power in the second quarter of 2010, said spokeswoman Susan Gallagher.

Iron worker killed in construction accident at Mohegan


Mohegan, Conn. Federal Occupational Safety and Health Administration joined Mohegan tribal safety officials today to investigate the circumstances of a fatal construction accident Tuesday.


An unidentified iron worker died from injuries sustained in a fall while working on the Mohegan Government and Community Center overlooking Route 2A, said Mohegan Tribal Chief of Staff Chuck Bunnell.The man was taken to the William W. Backus Hospital where he died from his injuries, Bunnell said.Bunnell said co-workers have described the man as a hardworking family man.

“We’re thinking of him and his family right now,” Bunnell said. Terry McEvily, OSHA’s assistant area director, said OSHA officials will conduct and inspection of the work site to ensure it complies with OSHA regulations.

Workers die on China rail project


At least nine workers have died in the collapse of a construction site on the Shanghai-Nanjing railway in eastern China, state media say.Four railway workers are still missing in the accident, which occurred in early morning in Jiangsu province, Xinhua news agency says. Initial reports suggested a gas explosion at a workers' dormitory had caused the collapse.China has ambitious railway plans but a poor safety record for workers. China plans to build more than 12,000km (7,500 miles) of high-speed railway to connect its major cities.New links in China's railway construction programme are part of the economic stimulus plan announced in November.It recently approved a further two-trillion-yuan ($300b, £218b) investment plan for its railway infrastructure over the next two years.It also plans a high-speed network, starting with a line between Beijing and Shanghai.Chinese-designed trains on this route will be capable of speeds up to 380km/h (236mph).

Electrician dies in Greene County accident


A 41-year-old man died yesterday after being thrown from the basket of a scissors lift that had been toppled at a construction site in the Poland Mine Site in Dunkard Township, Greene County.

Richard Reed Custer of Eighty Four was taken from the scene to Ruby Memorial Hospital in Morgantown, where he was pronounced dead on arrival.

The accident occurred around 11:20 a.m. when Mr. Custer, a journeyman electrician employed by TP Electric, was working for Lincoln Contracting at a water pumping/filtration plant construction site. He was on a scissors lift working overhead to install brackets when a forklift picking up wooden forms hit the scissors lift, causing it to topple on its side, throwing Mr. Custer from the basket.

An autopsy is scheduled today. State police are investigating.

Crash leaves three dead on I-12 south of Covington


Three people were killed and three others injured Sunday night when a motorist lost control of her car, drove across the median and collided with an SUV on Interstate 12 south of Covington, authorities said.

The crash near the construction site of the Colonial Pinnacle Nord du Lac shopping center between U.S. 190 and Louisiana 21 comes as preliminary work is being done to prevent crossover accidents on I-12 in St. Tammany.

Monica Cohn, 34, of Hammond was driving a Nissan Altima west on I-12 about 7:15 p.m. when she veered from the left lane into the right lane, State Police spokesman Trooper Louis Calato said. Cohn abruptly steered left, sending the car out of control and spinning across the grassy median and into the eastbound lanes, where it was struck by a Chevy Tahoe driven by Vincent Jeanfreau, 33, of Lacombe, Calato said.

The SUV crashed into the car, sending both vehicles spinning off the roadway, Calato said.

"The one thing we don't expect is a vehicle to come across the median into our lane of travel, basically out of nowhere, " Calato said. "It happens so fast and so unexpectedly I don't believe Mr. Jeanfreau could have done anything to avoid this crash."

SUV rolled over, ejecting Jeanfreau's wife, 27-year-old Amanda Jeanfreau, Calato said. She was pronounced dead on the scene. Cohn and her passenger, 47-year-old Hollis Ard Jr. of Gretna, were also pronounced dead at the scene, he said.

Vincent Jeanfreau and two children in the SUV were taken to St. Tammany Parish Hospital near Covington by Acadian Ambulance, Calato said. Vincent Jeanfreau and a 7-year-old passenger had moderate injuries and an 18-month-old passenger had minor injuries, he said.

Crossovers accidents are especially dangerous, Calato said, since they typically result in head-on collisions or direct crashes into the side of a car.

In February, the state Department of Transportation and Development began a $5.2 million project to install cable barriers along the 32 miles of I-12 through St. Tammany. These barriers, made of metal cables strung between posts, stop cars as they travel across the medians and are expected to be completed by the summer.

"This is our first pilot program to install them and see how they work, " said Dustin Annison, a spokesman for the department. "You could see them popping up in different areas of the state."

Medians in St. Tammany, and most of Louisiana, are about 64 feet wide, Annison said. Federal guidelines say barriers are unnecessary for medians wider than 50 feet, he said.

The transportation department, or the private firms it contracts with, will be responsible for maintaining the cable barriers, to ensure they are kept tense enough to prevent accidents, said Brendan Rush, with the department of transportation.

St. Tammany was chosen as one of the first sites for the new barriers after a study of crashes from 2005 showed the parish had the second-highest rate of crossover accidents in the state. The department has already installed barriers on Airline Drive in St. Charles Parish and plans to begin adding them to Interstate 10 in St. James Parish by the end of the year.

In 2007, there were 10 crossover crashes along I-12 in St. Tammany Parish, compared with 9 each in Tangipahoa and Livingston parishes and 1 in East Baton Rouge Parish, according to the state transportation department.

There is no clear reason why St. Tammany sees more crossovers, Annison said.

"Crashes are hard to predict; you often can't find any particular patterns with them, " he said.

Construction worker dies following crane accident at Total in Port Arthur

A construction worker at Total Petrochemical who was injured Saturday died today at Christus St. Elizabeth Hospital in Beaumont. About 4:15 p.m., the contract worker at the Port Arthur plant site suffered a head injury, when something apparently fell from a crane, according to Pat Avery, Total Port Arthur's human resources and communications manager.

He was flown by helicopter to Christus St. Elizabeth, where he died at 1 p.m. today, Avery said.
"We're still investigating the accident, and we don't know what happened yet," Avery said.

Total has not release the construction worker's name or the construction company who employed him. Avery was unsure whether the man was from Southeast Texas.The federal Occupational Safety and Health Administration will investigate the incident, Avery said, while Total operates its own investigation.

"We are deeply saddened by this, and our hearts go out to his family and coworkers," Avery said.

Van crashes into scaffold in Harlem, injuring 6;1 construction worker in critical condition


An out-of-control van jumped a curb in Harlem Saturday morning and slammed into a construction site, knocking down scaffolding and injuring six people.

One of the hardhats working on the scaffolding was critically injured and had to be dug out from a pile of twisted metal and shattered wood, witnesses said."I heard the collision and then I came out and saw the crash," a construction worker who was across the street. "We pulled the scaffolding off and that's when we saw him," said Consella, 33. "He was blue ... he was bleeding from the mouth and head."

Construction Accident Injures 11 In Harlem Pizzeria


Five people were hospitalized after a wall at a Harlem construction site fell into a next-door pizzeria Friday.

As workers poured concrete into an exterior wall at 270 West 123rd Street shortly after 3 p.m., the wall became so heavy that it fall into a next-door restaurant named Pizzeria 123.

Six customers and five pizzeria workers had minor injuries, but only five people sought medical treatment at St. Luke's Hospital.

Fire officials said a much bigger accident was averted.

"Luckily it wasn't heavily occupied at the time," said FDNY Deputy Chief Michael Mullins. "Could have been later in the day, could have had a lot of children involved. There were only approximately six or seven in the store at the time."

A 10-feet by 15-feet hole was left at the site of impact, and crews worked Friday evening to remove stray bricks and concrete.

"Where the hole is currently, that's a bearing wall," said Manhattan Councilwoman Inez Dickens. "So they have to be sure the support is there for the remaining floors of the building."

Manhattan Borough President Scott Stringer called for a full investigation of what went wrong at the construction site.

Department of Buildings officials were looking into the structural integrity of both buildings Friday, and issued a stop work order at the construction site.

Building site accident reporting ‘is flawed’

ABU DHABI // The reporting of accidents on construction sites is flawed, a new study says.The problems include a lack of basic information, such as the injured person’s nationality and the machinery being used, according to the UAE University study.The Ministry of Labour’s safety and inspections office plans next week to review how the information is collected.

“The system the ministry is using right now is kind of general,” said Noura al Kaabi, the lead researcher of the study. “The safety department is covering all kinds of occupational hazards so it’s not particular for construction. We designed a better, simplified computer database for the ministry that fits with the construction industry.”

She characterised the proposed reporting system as “a report and an investigation at the same time” because of the depth of information it allows.

“If we have a fall, you select where he falls from,” she said. “An elevated platform, a roof, an opening, a staircase or scaffolding? And then what kind of fall protection was in place – a guard rail system or safety nets? Was he pushed or was it a defective fall protection system?”

The director of the inspections department at the ministry, Muhsin Saeed, said yesterday that it has been in contact with Ms al Kaabi.

“Maybe we can mix her system and ours together,” he said.

Health and safety managers of major developers also urged the ministry to improve how it records construction accidents.

Dave Bass, of Al Naboodah Contracting Company, said that while builders were required by law to enter basic information such as the person’s name and sex, “there are also some strange questions there”.

“If you look to the labour law, there is a table that tells you one of the items you’re supposed to have within the reports is how much the guy was earning,” he said. “What on earth does that matter to an accident report?”

Andrew Broderick of Aldar Properties, one of the country’s largest developers, said that before yesterday he was unaware of any kind of form used by the ministry for occupational accidents.

“I’ve never seen this before in my life,” Mr Broderick said after receiving a faxed copy.

He said the form was inadequate for the construction industry. “I know that straight away because it asks for ‘profession or trade’, so this could be oil and gas, this could be working in a factory or a kitchen.”

Mr Broderick and Mr Bass stressed that the fundamental problem was a lack of enforcement.

“Do the little companies actually report their statistics?” he asked. “I know of some small companies of 200 or 300 people that build two or three villas at a time and they’re not reporting anything.”

Ms al Kaabi said one of the study’s objectives was to help the ministry determine which companies should be awarded contracts. Those with poor safety records could be ruled out.

The trade group Safe Build UAE recently published figures showing there were 20 fatalities last year and 690 lost-time injuries on construction sites. The most common cause of injury was falling from height, with 81 incidents.

2 hurt as crane crashes down on cars

JEDDAH: Two people were injured when a construction crane came crashing down on top of two passing vehicles and a government building during morning rush hour in Jeddah’s Al-Rawdah district yesterday.

The incident took place at 8 a.m. and the area was sealed off for more than six hours. Three Civil Defense rescue units were dispatched to the scene.

“I was taking my son to school when the crane came crashing down right in front of us, smashing several cars,” said Kariema Abou Baker, a 42-year-old Australian mother of six from Melbourne.

“I screamed and my son was terrified. I am still in a state of shock,” Kariema, who is married to a Saudi, told Arab News in a phone interview a few hours after the incident.

She added that the loud sound of the impact had people rushing out from the surrounding shops to investigate the cause.

“I feel sorry for those injured. I take the same route every day to drop off my son. It could have been our car crushed under that crane,” she said. “I am disgusted with the construction company and municipal officials who inspect the site.”

Civil Defense spokesman Capt. Abdullah Al-Amri said a construction worker and the driver of one of the vehicles were injured in the accident.

“The crane fell on two cars and a government building located on the opposite side of the road,” he said. The accident also damaged parts of the road.

“The crane was attached to the fourth floor of a building under construction,” said Al-Amri. “We are investigating what caused the accident,” he added.

“Either the crane was very old or it was overloaded,” Civil Defense Director in Jeddah Brig. Gen. Mohammed Al-Ghamdi told reporters at the accident site.

But a construction supervisor who did not want to give his name told Arab News that the crane was fairly new. “It was three years old and was scheduled for maintenance,” he said before rushing off not wanting to disclose further details.

The two cars damaged were a Lexus and a GMC suburban. The injured driver of the Lexus was taken to King Faisal Specialist Hospital for treatment.

Building sites safety blitz to cut deaths

Rochdale has been targeted by the Health and Safety Executive for an initiative to reduce the death toll in the construction industry.

Starting on Monday, HSE inspectors will be out in force visiting sites to drive home the safety message — especially in the refurbishment, repair and maintenance sector.

Around 1,500 construction sites — including 200 across the North West — are part of a campaign to tackle poor safety standards.

In 2007/08 there were 72 deaths on construction sites, with 38 in the refurbishment sector.

Inspectors will make unannounced visits to sites to raise awareness of the problem, with the refurbishment sector accounting for 50% of all accidents, but representing only 40% of all construction activity.

In a similar initiative last year, 187 inspections were carried out in the region with enforcement action being taken in 103 cases.

The HSE has warned that contractors can once again expect strong enforcement action.

Inspectors will be targeting principal contractors to see how they are managing working safely at height; good order on site and the risks associated with the removal of asbestos.

HSE’s chief inspector of construction Stephen Williams said: “While workers in the refurbishment sector continue to be injured and killed, HSE will continue to target the contractors and principal contractors on those sites which flout health and safety law and come down hard on them where necessary.”

Other boroughs involved in this first crackdown are Oldham, Barrow, Kendal, Lancaster, Morecambe, Macclesfield, Knutsford, Crewe, Nantwich, Congleton, Chester, Warrington, Northwich, Ellesmere Port and Neston, Runcorn and Widnes.

Deaths in Uganda building collapse

Six workers have been killed and a number of others are thought to be trapped after a building under construction collapsed in the centre of Kampala, the Ugandan capital, officials said.

Six other workers were taken to hospital with severe injuries after they were removed from the rubble, according to a local police officer.

A worker said the accident happened when a wall fell down early in the day on Thursday.

He said around 24 people were working on site when the building caved in.Rescue teams were digging at the site, trying to retrieve those trapped.

Ugandan civil engineers have complained about construction companies not following safety and labour regulations, exposing the workers and the public to unnecessary risks.

One dead, four injured at building site

ONE person died and four others sustained injuries yesterday when earth at a city construction site caved in on them. The accident, coming about 16 hours after a fire destroyed a city market, occurred at an excavated plot of land on Snay Bin and Dastur streets.

Witnesses said Edward Buwembo, Sande Omusingo, Matia Arinaitwe and two other unidentified men, were scooping soil out of a 10 metre deep hole, when earth suddenly hurtled down, killing Arinaitwe instantly.

Moments after the accident, men dashed to the scene with spades and rescued the other four. They received treatment at Mulago Hospital in Kampala. Arinaitwe’s body, was recovered and taken to the city mortuary.

The construction site is said to belong to Gaster Lule, the proprietor of Ntake Bakery. Kampala city council spokesperson, Simon Muhumuza, said Lule did not follow procedure like having a registered engineer on site.

Witnesses said the casual labourers had excavated murram for over a fortnight. Godfrey Byaruhanga, a survivor, said they were paid sh5,000 per day.

The Inspector General of Police, Maj. Gen. Kale Kayihura, who cut short a trip to the Nakivubo Park Yard Maket, visited the scene. “We must find out who authorises these defective plans.

We cannot allow this to continue happening. Whoever is in charge should be held responsible. We will investigate and the person found liable will be arrested,” he stated.

He said the Police would ask Parliament to give them authority to authorise such projects. Kampala resident district commissioner Alice Muwanguzi said the incident justified the Government’s desire to take over the management of the city. Lule could not be reached for a comment.

Last year eight people were killed and one injured after an excavated wall collapsed at the construction site of the NSSF pension building on Lumumba Avenue in Kampala.

Construction sites make deathtraps for kids


At least ten passers-by have died in accidents at construction sites in Ho Chi Minh City since last June but the contractors who neglect safety procedures have yet to receive even a slap on the wrist. Of the ten young pedestrians and motorcyclists to have perished at the transport construction sites, the youngest victim was four years old while the oldest was a college student.

Tran Hong Nam, deputy chief inspector at the Ho Chi Minh City Transport Department, said his inspectors had been doling out strict penalties for safety violations as part of daily inspections at construction sites around the city.

But the inspections have yielded little results and the strict penalties have come in the form of a few contractors paying out minor compensations to victims’ families.

After a seven-year-old street performer died after falling down a manhole at a Binh Thanh District construction site on New Year’s Eve, the Transport Department simply instructed the project’s investors, contractors and supervisors to check their manholes.

Department Director Tran Quang Phuong has said the body would issue specific policies for construction site safety, including penalties for violators, sometime this year.

He said the department would make it obligatory for all construction contracts in the city to stipulate exactly who would be responsible for safety and accidents.

Lawyer Truong Xuan Tam from the Ba Ria–Vung Tau Province Bar Association said individuals or organizations found responsible for such deaths could face criminal charges.

Uncovered manholes, weak barriers around construction sites or lack of a warning notice could all be used as evidence against contractors, he said.

He said those found guilty could be jailed for up to 15 years in line with item 220 of the Criminal Code, which regulates transportation construction sites.

He also said it would not be difficult to trace evidence back to those who violate safety regulations.Offenders could be prosecuted even when the victims’ families do not take legal action, he added.

Dangerous sites

Despite the efforts of the department, conditions at most roadside construction projects have not improved.

On the corner of Dien Bien Phu and Truong Sa streets in Binh Thanh District, traffic has to squeeze through a rough section road beside a ten-meter-deep hole dug for a sewer installation as part of the Water Environment Project.

The uncovered hole was barely surrounded by weak barriers that could easily allow pedestrians or cyclists to slip through.

There are several similar holes on Truong Sa Street, which stretches through Binh Thanh and District 1, and on Tran Van Dang Street in District 3. There are careless makeshift fences and no warning notices at any of the sites.

Many Water Environment Project contractors have also failed to properly pave streets after finishing construction projects along the Nhieu Loc – Thi Nghe Canal in District 3, leaving behind a number of dangerous potholes and rough sections of road.

Several sections of Tran Van Kieu Street in District 6 are even worse with no barriers between the street and a major construction site where potholes are up to a meter deep. The streets are often filled with water and mud spilled by construction trucks.

A recent case

The body of a seven-year-old street performer was found inside an uncovered manhole on Cu Lao Street in Phu Nhuan District this New Year’s Day.

Local residents said they found the body of Ngo Hoang Vo floating in the sewer at around 2 p.m. on January 1.

Police said Vo had been performing with his nine-year-old brother Ngo Thanh Van since 6:30 p.m. on New Year’s Eve.

Vo had fallen into the manhole after the brothers had split up to perform at different eateries. The manhole had been left uncovered by contractors working on the city’s Water Environment Project, police said.

Preliminary investigations suggest site of trench collapse may not have been officially registered

Five men have been killed on a South African construction site after a trench collapsed.Inspectors are currently probing circumstances surrounding the incident, but it is believed one worker was in the five-metre-deep trench when the excavated sand fell on him.

Four of his co-workers tried to help him out, but the entire trench then collapsed on them.The five victims had been digging the trench to install sewerage pipes at Ntha township, near Pretoria, in the Free State.Work at the site was halted following the accident.

Preliminary investigations have found the employer had not registered the construction site with the department of labour, according to reports in South African newspaper, The Citizen.

Health and safety investigation is launched into fatal concrete collapse in Aragon

Two builders have been killed in an accident on a Spanish construction site, which left a number of others with injuries.

One Romanian and one Portuguese man died on the secondary school site in Teruel, when several sheets of reinforced concrete gave way.

Another Portuguese builder was left with a fractured leg and other workers had to be treated for minor injuries. It is believed some of the witnesses also had to receive psychological help.

The victims had been spreading sand below the structure, directly below the concrete sheets. The worker who suffered the fractured leg had been working on top of the structure when it fell.

An investigation into the incident was launched immediately by the independent Aragon Institute of Health and Safety in the Workplace.

Four people have now been killed on construction sites so far this year. Spanish trade unions and Aragon are now calling for a thorough investigation and more measures to prevent accidents such as this.

Seven dead in Chinese tunnel collapse

Fourteen other workers missing after incident on Hangzhou subway project.The death toll has risen to seven at a construction site in eastern China, where a tunnel collapsed on Saturday.

Fourteen other workers are still missing following the incident in Hangzhou, Zhejiang province, in which a 75m section of a subway tunnel collapsed.

Around 50 workers and 11 vehicles were trapped in the rubble following the accident on Saturday afternoon. An investigation is under way and all subway construction works in the city have been suspended for safety checks.

The head of the rescue effort, Wang Guangrong, said that the chances of finding further survivors were slim following flooding at the site from a nearby river. Water levels in the crater had reached 6m, he said.

More than 1,000 police and firefighters have taken part in the rescue effort in Hangzhou, where the first phase of construction of a subway network is in progress.

The £3bn project is set to be completed in 2011 and will run from the north-east of the city to the south. Hangzhou wants to build eight subway lines by 2050.

Workers buried in rubble after 22m-deep pit collapses at chemicals research centre construction site.Three building workers have been killed and a further eight injured after a collapse at a construction site in Korea.

The men were buried under soil and debris when a metal beam buckled at a chemicals research centre in Pangyo on Sunday, causing six container boxes to fall 25m.

Eleven workers are thought to have become trapped under rubble in a 22m-deep pit at the site on the southern outskirts of Seoul.

One of the injured workers, 66-year-old Cha Seung-dong, told Korean newspaper JoongAng Daily: “When we were about to lay pipes at the excavation site, earth crumbled from the northern side. I ran in the other direction, but lost consciousness after being hit in the neck by a collapsing steel frame.”

Around 150 policemen and firefighters spent five hours trying to rescue workers trapped under the rubble.

The building, which is being developed for SK Chemicals as part of a new Seoul satellite city, began construction in September. It is thought the accident may have been triggered by rain and warm weather softening the ground around the site.

The scheme's main contractor, SK Engineering & Construction, said the company would “do its best to give medical treatment and compensation to the victims as well as to find the accurate cause of the accident”.

Two builders, one Rumanian and one Portugese, died yesterday in an accident on a building site at the 'Segundo de Chomón' secondary school in Teruel when various sheets of reinforced concrete gave way.

As well as the fatal victims, the accident also left another Portugese builder in hospital with a fractured leg.

A number of other workers on the building site had to be treated in situ for minor injuries and some of the witnesses to the accident had to receive psychological help.

The concrete sheets fell directly onto the two fatal victims, who were spreading sand below the structure at the time. The worker with the fractured leg was working on top of the structure when it fell.

An investigation into the accident has been launched straight away by the independent Aragon Institute of Health and Safety in the Workplace.

This accident takes the construction site death toll to four so far this year in Aragon and trade unions are calling for a thorough investigation and more measures to prevent accidents of this kind.

Adolfo Barrena of the Izquierda Unida party in Aragon, denounced the incident, saying that yet again people have lost their lives in the course of doing their jobs and called for 'urgent, resolute and decisive measures against accidents in the workplace.

Scaffolding contractor cited in fatal collapse

— A scaffolding company caused the collapse of a wooden walkway cover in downtown San Diego that killed one man and injured at least 14 others in August, a state agency has concluded.

A report by the California Division of Occupational Safety and Health said Paramount Scaffold Inc. created a hazard when workers stacked scaffolding planks and metal frames against the pedestrian canopy.

The company was sent a notice that it will be fined $10,120 for two serious violations of state safety regulations.

The 200-foot-long walkway cover, along Imperial Avenue at 15th Street in East Village, collapsed on pedestrians Aug. 28. Fifteen or 16 people were hospitalized, and one, Tyrone Allen, 49, died Feb. 12 of injuries he suffered that day.

Some of the injured were homeless people who lived at St. Vincent de Paul Village, across the street from the construction site. The Cal-OSHA report said some shelter patrons used the walkway as a refuge.

The cover was erected to protect pedestrians from debris from the construction of a four-story apartment complex called Studio 15, an extension of St. Vincent de Paul Village being built by Affirmed Housing Group of San Diego.

Cal-OSHA began an investigation within 40 minutes of the accident, according to its report, issued yesterday.

The report said four Paramount Scaffolding workers stacked planks and frames against the walkway canopy about 8:15 a.m. About 11:30 a.m., a crane operator saw the walkway bowing beneath the weight of eight to 10 planks. He warned another workman, who told the scaffolding crews and his supervisor.

Security cameras showed the walkway start to lean, then suddenly buckle and collapse at 12:20 p.m. Several people scattered amid the debris; others were trapped.

The actions by the Paramount Scaffolding workers violated two state safety regulations against overloading scaffolding and creating a hazard with stored material, Cal-OSHA concluded.

The report categorized the violations as “serious.”

The company has a right to contest the fines.

“We haven't seen it (the report), at least not as of right now,” Jim McCormick, chief financial officer of Paramount Scaffold Inc. in Los Angeles, said yesterday. “We talked to Cal-OSHA a week ago, and they told us it was two weeks away.”

San Diego attorney Todd Neal, representing Allen's widow, Tina Allen, and the couple's three adult children, filed a lawsuit within days of the collapse. Allen's injuries instantly made him a quadriplegic, Neal said.

The lawsuit alleged negligence, but because Allen has died, it will be amended to claim wrongful death, Neal said.

Named as defendants are property owner Studio 15 Housing Partners, general contractor Allgire General Contractors Inc. and Paramount Scaffold. Allgire and Studio 15 could not be reached for comment late last night.

Neal said it was significant that the scaffolding workers were warned that the walkway was beginning to buckle.

“They did nothing,” he said.

The construction worker who died at the new Terminal 3 roadways project at McCarran International Airport on Monday has been identified as 25-year-old Byron "PK" Souza of Las Vegas, the Clark County Coroner's Office said today.

The cause and manner of death has not been determined, a spokeswoman for the coroner's office said.

Souza died at 11:30 a.m. Monday at Sunrise Hospital and Medical Center. He was a laborer working for Las Vegas Paving Corp., said Randy Walker, Clark County director of aviation.

Reports indicate Souza was working in a ditch at the terminal roadways site at about 11 a.m. Monday when the operator of a Bobcat earth mover lost control of his vehicle and ran the worker over.

The reason the operator lost control of the machine has not been determined. The incident is under investigation by the Nevada Occupational Safety & Health Administration.

Worker at Construction Site Dies

AUBURNDALE | A 42-year-old Auburndale construction worker died Thursday at the city's wastewater treatment plant when he was pinned between a concrete wastewater junction box and the bucket of a trackhoe.

Mark Ingram was assisting co-workers in preparations to lower the hollow junction box into the ground about 8:30 a.m. when the trackhoe shifted, crushing him against the concrete box.

Ingram, who was working for Indian River Industrial Contractors, died at the scene, said Auburndale police Lt. Thrasey Tucker. "This was purely an accident," he said. "From a law-enforcement perspective, there is no foul play, nothing of a criminal nature."

Tucker said crews were using a trackhoe, which is similar to a backhoe but with tank-like tracks instead of tires, to lower the junction box into the ground. The box was at ground level next to the hole. A co-worker left the cab of the trackhoe to help Ingram, who was hooking cables connecting the junction box to the trackhoe.

"The trackhoe kept moving," Tucker said, "and the bucket pinned (Ingram,) crushing him."

Investigators with the federal Occupation Safety and Health Administration initiated an independent investigation Thursday to see if the company violated any safety standards. Tucker said no city employees were involved in the construction project at the plant at 890 Braddock Road.

The city is doubling the capacity of its wastewater treatment plant from 2 million gallons per day to 4 million gallons.

Arreza said the forklift driver, Menti Dacanay, has been held at the site.He said Dacanay began his shift at the shipyard's Assembly Shop C at 8 p.m. Saturday.Work at the shipyard has been on a 24-hour basis. The Korean shipbuilding firm has finished four of the 13 ships it is scheduled to build until 2013. Arreza said Baek's death "appeared accidental."At the shop, forklifts are used to move metal plates and ship components, he said.Baek’s is the first known accidental death among Hanjin’s Korean employees. SUBIC BAY FREE PORT—A 51-year old Korean on Sunday became the latest fatality in the string of accidents that has hounded the shipbuilding facility of Hanjin Heavy Industries Co.-Philippines (HHIC-Phil) since the $1.7-billion shipyard began operations in 2006 at the Redondo Peninsula here.

Choi Dong Baek, who was employed as a foreman by Greenbeach Power Corp., a subcontractor of HHIC-Phil, died in a hospital in San Marcelino, Zambales, at about 1:30 a.m. on Sunday after a forklift ran over him at the Hanjin facility’s Assembly Shop C.

Choi’s death came just two days after another Filipino worker, 19-year-old Raldon del Rosario, perished at the shipbuilding site when an 800-kilogram curtain door assembly toppled while being tested by Great Steel Construction, another subcontractor.

SBMA investigators, who interviewed witnesses at Sunday’s accident site, reported that the incident happened at about 1:50 a.m., as workers were cleaning up after a basketball game during the midnight break.

Choi reportedly picked up the ball and was kicking it about like a soccer ball, when it rolled onto the path of the moving forklift.

Menti Dacanay, a Filipino rigger working for free port I-Tech Corp., also a subcontractor, was driving the forklift to replace the movable basketball stand.

Choi reportedly tried to retrieve the ball, but he was hit by the maneuvering forklift, which subsequently dragged and ran over the victim.

Investigators noted that Dacanay was not licensed to drive the forklift, while Pyeong Jong Yu, deputy managing director of HHIC-Phil, said Dacanay used the equipment “without getting any instruction from either Korean or Filipino foremen.”

“We are still trying to verify how he got the key of the unit, since the key should have been kept in the toolbox container,” Yu said in an accident report sent to the SBMA.

Choi became the 19th fatality at the Hanjin shipyard, where accidents¬—15 of them construction-related—had so far claimed the lives of 18 Filipino workers since 2006.

เม็กซิโกที่ตัวน้อยกว่ากลับบุกกดดันใส่ไทยมากขึ้น ช่วงทดเวลาเจ็บนาที 94 เกิดเหตุวุ่นวายในสนามขึ้นเมื่อ มาร์โก วัลเวอร์เด ขึ้นโหม่งชนกับ อัมรินทร์ เยาว์ดำ ก่อนจะล้มลงมาแล้วนักเตะจังโก้ไปชักศอกใส่นายประตูของไทยจนปากแตก ทั้งสองคนเลยฟาดหมัดใส่กัน ทำให้ พอล วอร์ด ผู้ตัดสินชาวแคนาดาตัดสินใจให้ใบแดงทั้งคู่ออกสนาม

เป็นของ วิศรุต พันนาศรี และ เดเนียล โรเจส ก่อนจะกลับมาเตะกันใหม่และเป่านกหวีดจบเกม ทีมไทยชนะไป 1-0 สร้าง ประวัติศาสตร์ผ่านเข้ารอบรองชนะเลิศเป็นครั้งแรก แต่หลังจบเกมก็เกิดเหตุวุ่นวายอีก เมื่อนักเตะทั้งสองทีมกรูจะเข้าชกกัน ทำให้เจ้าหน้าที่ต้องมาเคลียร์กว่า 10 นาที จนสงบและนำนักเตะเม็กซิโกออกจากสนามได้
